Health Assessment & Planning: Creating Accountability Through Community Leadership

This conversation, hosted by the California Accountable Communities for Health Initiative (CACHI), will explore how Accountable Communities for Health (ACHs) are serving as bridges between communities, Managed Care Plans (MCPs), and Local Health Jurisdictions (LHJs), and why community leadership is essential to shaping Community Health Needs Assessments (CHNAs) and Community Health Improvement Plans (CHIPs).

The session will feature speakers from two ACHs, Ventura County Community Health Improvement Collaborative and Sonoma County’s Health Action Together, alongside MCP and LHJ leaders.
