Vision Award for Excellence in Health Planning: Eligibility Requirements

A nominee must have made a significant and noteworthy contribution to the practice of health planning through professional effort, personal work, or voluntary service, which demonstrates excellence as judged by his/her peers. This nominee must be a member of the American Public Health Association, Community Health Planning and Policy Development Section.

The award may recognize sustained efforts over a period of time or a single outstanding contribution. The accomplishment must be practice-oriented and demonstrate excellence, creativity, and innovation in the application of planning methods that address a significant health-related issue in a practice setting. Research projects and work dealing with planning theory are eligible for recognition if they have been applied in practice. One award may be made per year. In very unusual circumstances, two awards may be made.
