Providing Proof of Status

 

Student: You may provide one of the following types of documentation as proof of status:

  • Official or Unofficial Transcript
  • Letter from Advisor
  • Letter from Registrar

Documentation should confirm that you meet the Student requirements:

  • enrolled in a degree-seeking program. 
  • taking at least six credit hours (undergraduate degree) or three credit hours (graduate degree) per semester or comparable credits in a quarter system. 

Student membership is available for up to six years per degree.

Early Career Professional: You may provide one of the following types of documentation as proof of status:

  • Copy of Degree/Diploma
  • Official or Unofficial Transcript 

Documentation must include the name of the college or university you attended and the date of graduation.

 

Special Health Worker: You may provide one of the following types of documentation as proof of status:

  • Letter from Employer
  • Copy of Pay stub
  • Copy of W2 Form
  • Copy of Unemployment stub

How to submit proof of status:

 

Proof of status may be uploaded when submitting your online membership application. You may also send your proof of status via email or fax it to 202-777-2533.
