The Conference will showcase programs dedicated to collaboration among health professionals, patients, and families and offer examples of excellence and innovation relevant to a variety of settings: primary care, medical home, and ambulatory specialty care; hospice, home care and other community-based programs; mental health and public health agencies, hospitals and health systems; rehabilitation and long-term care; schools of medicine, nursing, allied health, and other health professions; and patient- and family-led organizations.
Continuing education will be available for physicians, nurses, and social workers. Others will receive a Certificate of Attendance.
